Botox® is proven most effective before the age of sixty-five and is not indicated as a method for improving sagging skin or damage caused by the sun. That’s why we are here - to discuss your options and create a beauty plan designed just for you.

Botox®, approved by the Food and Drug Administration (FDA), is a safe muscle relaxing agent derived from Botulinum that boasts many varied and growing applications. Although it is most widely known for its effectiveness in reducing wrinkles and fine lines; it is also used to treat ophthalmological and neurologic disorders, in the prevention of migraine headaches and even to stop excessive sweating.

Preparation:

There is no preparation for Botox®.

Procedure:

Got a few minutes? That’s all it takes to realize the anti-aging results of Botox®.

First, your healthcare provider will determine where the muscles that need to be treated are located. You may be asked to smile or frown to illustrate the natural movement of your face. A topical anesthetic may be applied, although most individuals do not require any, as there is little or no discomfort associated with the procedure.

Using a tiny micro needle, an extremely small amount of Botox is precisely injected into your face in the designated muscles. Each area of your face takes approximately five minutes to treat.

Aftercare:

With Botox, there is virtually no aftercare required. You may cover any minor temporary bruising that may occur with makeup and continue with your daily schedule. You can drive yourself to and from your appointment. For best results, it is recommended that you do not drink alcohol and that you remain upright for several hours after your treatment.

You may experience a temporary headache or irritation at the injection site. Additional side effects are rare. Discuss any conditions or concerns with one of our healthcare providers.

Results:

You can expect to see results within a few days after your procedure. These results will begin to fade over three to six months, however, you can safely repeat your Botox® treatment two-to-three times a year to maintain the look you desire. The best news is that Botox® actually prevents new lines from occurring. With continued use, your face will begin to “forget” to wrinkle and over time you will see that you require less treatments, less often.

Precautions:

Possible side effects vary depending on the muscles being treated. In rare cases, patients have reported a temporary weakness in muscles near the injected area. Also in rare cases, there can be a drooping of eyelid or asymmetry of facial expression. But as the effects of Botox® are temporary, any adverse side effects would also be short-lived.

■ CHEMICAL PEEL

Put your best face forward.

Rejuvenating Chemical Peels have been used for over a century to remove dead skin cells and stimulate the production of new ones. A chemical peel is indicated when you are looking to:

  • brighten your skin
  • remove spots or light freckling
  • minimize fine lines
  • even pigmentation
  • smooth shallow acne scars
  • erase sun damage
  • control active acne

With age, the natural rejuvenation process of skin cells slows down. A chemical peel facilitates renewal, making skin appear tighter and less wrinkled, leaving you with a more youthful glowing appearance.

A chemical peel is often performed in concert with a facelift as it improves texture and tone while a facelift targets loose or excess skin. A chemical peel is not indicated as an alternative procedure to the facelift.

Depending on the goal for your peel, we may use one chemical, such as glycolic or trichloroacetic acid, hydroquinone or a combination of chemicals. The solution used defines the chemical peel as a light or superficial peel, medium peel or deep peel. The doctor, or one of our highly trained, licensed medical estheticians will determine the best strength and combination of chemical agents for your skin.

Preparation:

You may be asked to use a skincare regimen leading up to the day of your Chemical Peel in order to prep the skin prior to the procedure for maximum result.

Procedure:

Unless you are having a very deep peel, you will not require any form of anesthesia.

During your treatment, a chemical solution is applied to the affected area, which, in the case of a Chemical Peel, is usually the complete surface of the face. The chemical agents immediately begin to peel any dead skin from the outer surface layers of your treatment area.

During the application, you can expect to feel a cool, warm or hot sensation as well as some stinging that can last for 10 minutes or more, depending on the strength and length of your peel.

Aftercare:

Because the strength of Chemical Peels vary, it is important that you follow your specific aftercare directions closely to encourage healing and minimize any risk of complications.

After a light peel, you should expect:

  • Some redness
  • Possible scaling that lasts a few days

After a medium or deep peel, you should expect:

  • Possible swelling and blisters
  • Blisters will break and form a crust and turn brown
  • Peeling which can last 5-8 days
  • Ointments and creams may be applied to keep the skin moist

After medium or deep peel, your new skin will have the appearance of a sunburn. The redness will become pink over time and could take up to several weeks as it heals. You can apply makeup after approximately 7 days. Every individual heals differently and we are here to guide you in your particular aftercare regimen.

As a rule, after all Chemical Peels, it is best to avoid sun exposure and apply sun-block with an SPF of 15 or higher at all times before going outside. Avoid makeup until your skin is healed enough to wear it. Again, we are here to help you determine the right progression for you based on your individual healing.

After a peel, it is not uncommon to experience a temporary breakout as the chemical agents that have been applied begin to generate new cells and your skin adjusts and heals.

Your skin is very sensitive at this time. Take good care of it and you will be rewarded by its rejuvenation. Be sure to let your skincare provider know of any concerns you may have throughout the healing process.

Results:

This is the part where people tell you how great you look; that your skin is glowing. That’s because it is. After a Chemical Peel, the surface of your skin is new. It will be tighter and smoother.

The result from your peel can last for months and you may want to follow-up with a series of treatments to maintain that fresh-face appearance. For many who want to fend off the advancement of fine lines and age spots, a long-term regimen of Chemical Peels is the perfect plan. ■ EARLOBE REDUCTION

Earlobe Repair and Reduction
Either through earlobe repair or reduction we can change what age, genetics or those long heavy dangling earrings have done to your ears. Earlobes are often lengthened and damaged over time and aesthetic earlobe reshaping can help you achieve a more youthful appearance.

It is believed that we have our fully developed ear size very early in our lives; therefore genetics play a large role in how our ear is shaped and sized. The most common cause of our ear disorder is the trauma we ourselves cause to them. And lastly, as we age our ears lengthen with gravity pulling in a downward position. Our goal is to restore that form and function to your earlobe; to alter the shape for a more aesthetically desired look.

During your initial consultation, Dr. Seiger will discuss the changes that you would like for your particular problem. You should discuss in detail your concerns so the appropriate aesthetic results can be performed. Prior to your procedure, a complete health history will be performed and specific instructions will be given to assist you in preparation for your procedure.

Earlobe surgery, for the most part has no risk involved. Complications are infrequent, however, as with any surgical procedure there are certain potential risks and complications that will be reviewed with you prior to your surgical procedure. The appropriate technique will depend on the nature of the specific problem to correct. Earlobe surgery will take 30-90 minutes to complete and is done using local anesthesia and a relaxing sedative. Earlobe surgery is often used in conjunction with other facial surgery techniques such as our One Hour Facelift.

Earlobe Repair
This is a simple, common, cosmetic procedure of torn earlobes. The procedure is done with local anesthesia and takes approximately 30 minutes to perform. Elongated slits and often complete split lobes are repaired. A special incision is made to repair the torn lobe and sutures are placed and remain intact for 5-7 days. The surgical site must be allowed to heal for approximately eight weeks before re-piercing can be completed. After the healing, very minimal scarring remains allowing for an aesthetically pleasing end result.

Earlobe Reduction
This is a procedure designed to remove excess skin and tissue from a small incision that blends into the ears natural creases. The procedure is done with local anesthesia and takes approximately 60 minutes to perform. Dr. Seiger will remove the skin and suture the lobe area leaving only a faint scar in the back of the earlobe allowing for the aesthetic reshaping and ear rejuvenation. The technique used will depend on your ears current shape and the result you wish to achieve.

Earlobe Wrinkling
This condition can be corrected by above procedures and also by laser treatment. This will aide in restoring smooth and youthful texture to the earlobe.

Recovery after Earlobe Rejuvenation
With earlobe rejuvenation you may be placed in a head dressing for compression. You will be given specific instructions for your individual care and suture removal. You will be up and around within a few hours and can resume your activities of daily living. You will complete your prescribed antibiotic and a mild pain medication will be prescribed. Your ear will be somewhat sore. With realistic expectations, you can expect your ear surgery to produce pleasing aesthetic results.

■ ENDOTINE BROW LIFT

The ENDOTINE, also known as a brow lift or "Power lift", is a surgical cosmetic procedure to remove wrinkles and folds from the forehead. This offers a novel and effective solution for fixation of the scalp tissue, completing the endoscopic forehead lift procedure. The ENDOTINE features suture-less MultiPoint Technology (MPT), a breakthrough platform technology designed to facilitate minimally invasive procedures, reduced operating time and improve surgical outcome.

The procedure is not time consuming and requires only the following steps:

• A bilateral 1-2cm incision is made in the frontal hairline
• The drill site is selected and the insertion site is made
• The ENDOTINE is implanted
• The brow tissue is elevated to the desired position
• The incision is sutured closed

The patient will be given specific post-operative and icing instructions.

■ EYELID (UPPER AND LOWER) SURGERY

Eyelid surgery, or Blepharoplasty, is one of the most common procedures performed for men and women looking to retain their youthful appearance.

It seems that the first thing you notice in the mirror is that you do not look rested in the morning; that the appearance of being tired begins too soon and lasts too long, sometimes throughout the day. Although common with age, in some cases, sagging or puffy eyelids develop early on.

The goal of eyelid surgery is to remove excess skin and fat, correcting sagging eyelids, diminishing fatty pouches beneath the eyes and removing excess skin from upper eyelids. An eyebrow lift is often performed simultaneously, as eyebrows often droop at the same time as the eyelids begin to change. Some patients opt to have a facelift or laser skin resurfacing with their eyelift, depending on what they want to achieve. At Skin & Vein, we work with you to be sure you know all of the options available to realize your greatest result.

Preparation:

Photos will be taken before your treatment to document your results.

Procedure:

Eyelid lifts are generally performed under local anesthesia.

For problems of the upper eye, Dr. Seiger places fine incisions in the crease above each eye and removes a thin crescent of excess skin and muscle. The incisions are then closed with very fine sutures.

To remove the fatty deposits under the eyes, Dr. Seiger places fine incisions just inside the lower eyelid and the fat is then excised.

Incisions are strategically placed in the natural folds and creases of the upper and/or lower eyelids and will become invisible or camouflaged over time.

You may require only upper or lower eyelid surgery. Some people benefit by having both procedures performed at the same time. Depending upon the extent of the procedure, surgery can take from 1 to 2 hours..

Aftercare:

After an eyelid lift, you will be instructed to keep your head elevated to minimize bruising and swelling. Cold compresses periodically placed on the eyes will also be helpful in alleviating swelling and discomfort. After about 7 days, make-up may be worn, if desired.

Results:

The more youthful, softer appearance you wished to see in the mirror returns. The hairline scars disappear. Friends and family are wondering where you went for vacation. You may even enjoy a wider field of vision if you had excessive skin that could have been blocking your view removed.

What does hair restoration or transplantation involve?
Hair transplantation involves taking mini (four to seven hairs) or micro (one to three hairs) grafts from the back and sides of the scalp and replacing them into the bald area. What many patients do not realize until speaking with surgeons from Skin & Vein is that the hair from the back and sides of the head is genetically programmed in the root to grow for a lifetime. This means that hair restoration is a permanent form of treatment. Just as important, the donor site from which the transplanted hair was taken will not become bald. Once relocated to the balding area, transplanted hair will retain the same color, texture and growth rate as the area from which it was taken.

Will it look natural?
Smaller grafts are spread out more evenly, thus giving a natural-looking hairline. Surgeons at Skin & Vein closely study each patient's hair topography. This varies with each patient, depending on the extensiveness of hair loss. One hundred to 800 grafts of various sizes can be transplanted at each session, which takes up to three hours to complete. During your initial consultation, a surgeon from Skin & Vein will estimate the anticipated number of sessions.

 

How long does it take for the transplanted hair to grow?
Transplanted hair needs time to "rest" after surgery. It will usually shed after four weeks with new growth appearing approximately 12 weeks later at a rate of 1/4 inch per month. If more than one session is required, we typically schedule patients to come back for their second session in three months.

How long will the procedure last?
Unlike topical treatments in which the user must apply the treatment on an ongoing basis for hair growth to continue, transplanted hair lasts a lifetime.

Is the procedure painful?
Hair transplantation is relatively painless. Using a local anesthetic, the procedure is performed by skilled surgeons at Skin & Vein in the comfort of our office. Patients remain fully alert throughout the surgery and can usually return to work within 24 hours. Mild discomfort may occur after the procedure when the anesthesia wears off. This is easily remedied with a mild analgesic.

Is there any risk involved?
With any surgical procedure, there is some risk involved. Fortunately, complications are rare and can usually be avoided with adherence to the pre-operative instructions and a thorough medical review of all patients. Some of the complications that may occur, but are rare, include: 

• Bleeding - which is easily controlled by pressure
• Infection and Swelling - which are medication controlled
• Temporary Numbness - which will resolve typically within three to six months.

Technologically advanced, Laser Hair Removal can treat small to large areas of your body. That upper lip or bikini line, underarms, back, or leg hair can be lasered away safely, quickly, and - perhaps best of all - gently. Razors, waxes, crèmes and electrolysis (which uses a much slower process where a needle is actually inserted into each hair follicle followed by an electric shock to burn the follicle), are no match for the quicker and far more comfortable laser, which uses cool light to quickly scan and treat the desired area.

Since hair grows in cycles and Laser Hair Removal treatment only captures hair that is growing at the time of the procedure, a series of treatments is required. Along with your hair’s growth which varies due to age, weight, hormones, diet, medication and metabolism -- many factors dictate the amount of recommended treatments; the area you wish to have treated, hair density, hair color and ethnicity.

Generally, African Americans or Hispanics require a longer series of treatments than those individuals with lighter skin.

Our very experienced laser techs (we've been doing it for many, many years) can give you an estimate of the number of treatments it will take to realize your desired outcome. We use the absolute best laser around for laser hair removal - the Lumenis, Lightsheer laser.

Please note: As of this writing there are no lasers today that effectively treat non-pigmented hair (white, blond, grey or red). If someone tells you they do, insist on speaking to their "light-haired" patients who had it done.

Procedure

Your Laser Hair Removal begins with a cleansing of the area that will be receiving treatment. Your technician will then shave the area before scanning it with the laser. As the laser moves over the treatment area, energy passes through the skin and is absorbed by the pigment in the hair follicle. All of the treated hair follicles are disabled in less than a second. Depending on the size of the area being treated, the procedure can take from a few minutes to a few hours.

It is a common misconception that you should let your hair grow out before receiving Laser Hair Removal. Although you should not tweeze, wax or use depilatory creams 2-3 weeks prior to your laser hair removal treatments, it is recommended you shave on the day of your appointment.

Aftercare

At the end of your laser procedure, the treated area is cleansed and you can return to your normal activities.

Sunscreen should be used on any area exposed to the sun. A special skin care line or routine may be recommended for your specific skin type as well.

Redness and swelling around the hair follicle of the treated area 20 minutes to 24 hours after treatment is a normal part of the treatment process.

Precautions

Those patients who are exposed to the sun, use tanning beds or spray tan, take medications such as Retin-A or antibiotics, or have recently had a chemical peel are more vulnerable to more serious side effects such as hyper and hypo pigmentation or scarring. Procedure

The laser peel is an outpatient procedure lasting between an hour and an hour and a half.

A local, topical anesthetic is applied and allowed to sit for approximately half an hour in order to numb the surface of your skin. Although discomfort is generally mild, a mild sedative may be given or recommended prior to the procedure.

During the procedure, a laser, moved over the surface of your skin, emits an intense beam of energy. This beam heats and vaporizes the tissues of the outer layer of your damaged skin upon contact, leaving neighboring, healthier tissue unaffected by the laser.

You can expect to feel a sensation much like sunburn, immediately following.

Aftercare

For more aggressive laser treatments you may be given a mild sedative and therefore need a driver to assist you home.

You can expect to experience redness and some swelling. You will be given directions for caring for your tender skin which may include ice, cleansing, and topical products to aid in healing. Depending on the type and depth of your treatment, expected downtime can be as little as one week, but may be longer. With lighter peels, you may experience pink skin for up to two weeks. With deeper peels, the skin may remain pink for up to four months. In all cases, your skin can be easily covered with makeup. We will give you a realistic idea of the of healing process and time you will need to plan for in regards to your specific treatment.

As with any treatment of the skin, avoid the sun and use sun-block at all times.

■ LASER TREATMENT OF FACIAL VEINS

Are laser treatments safe?
Yes. Lasers have been used in medicine for a wide range of treatments since the early 1960s by many specialties. In the hands of a trained healthcare professional, the use of a laser is safe and effective.

How does the laser work?
A laser generates an intense beam of light. This beam brings energy to a specific site, through a small hand piece connected to the laser.

The laser light is absorbed by oxy hemoglobin (bright red blood cells carrying oxygen) and melanin/pigment (black or brown pigment found in the skin) causing decomposition or destruction by heat of unwanted cells while leaving healthy cells intact.

What is a vascular lesion?
A vascular lesion is formed by abnormally large or numerous blood vessels located directly under the surface of the skin. These vessels may be visible through the skin or result in a red appearance of the skin. Spider veins (telangiectasia), shown below, are the most common vascular lesions.

What is a pigmented lesion?
Melanin is the dark pigment present in skin and is produced by melanocytes. Pigmented lesions occur when an abundance of melanocytes are found in the skin. When dark pigment multiplies, it forms freckles, brown age spots and moles. Lentigines, pictured below, are an example of a pigmented lesion.

What should a patient expect during and after treatment?
Treatment may vary, depending upon patient characteristics and the area being treated.

You will be asked to wear special goggles to protect your eyes from the intense beam of laser light.

You will be placed in a reclined position, depending on the location of treatment. The technician will use a pencil thin hand piece to direct a beam of laser light to the lesion. In most cases, anesthesia is not required.

Some patients report feeling a slight stinging sensation during the treatment followed by some redness which usually goes away in 2-4 hours. Some swelling may also be experienced and may last 1-2 days. There will not be any purple skin discoloration or bruised appearance to the treated skin.

Postoperative care is minimal. Your doctor may ask you to use an anti-bacterial ointment on the treated area for a short period of time and will suggest that you stay out of the sun during the healing process.

Final treatment results should be evident within 10-14 days after treatment for vascular lesions and within 2-3 weeks after treatment for pigmented lesions. In the meantime, normal daily activities can be resumed with only a slight redness evident at the treatment area.

What does the laser look and sound like?
The laser is small - about the size of a VCR. As pulses of energy leave the end of the hand piece fiber, you will hear a soft beeping sound.

Injectable Fillers

This is the most often asked about treatment and there are many injectable fillers to choose from. In most cases Dr. Seiger's injectable of choice is Restylane. Other product names you may be familiar with include: Perlane, Juvederm, Radiance, Captique, HylaForm, Artefill and Evolence. Depending on your desired result, and the shape and condition of your lips, Doctor Seiger will recommend the best injectable filler for you.

Preparation:

Before treatment, you will be instructed to discontinue use of any non-steroidal anti-inflammatory medications, aspirin, St. John's Wort, or high doses of Vitamin E supplements (as these may increase any bleeding or bruising at the injection site. Be sure to tell us if you have previously suffered from facial cold sores as a medication may be prescribed to minimize recurrence.

Procedure:

An anesthetic cream may be applied to your lips to numb them, reducing any discomfort from the treatment. A very small needle is then inserted and a very small amount of filler is injected into your lips to achieve the desired effect.

After any minor swelling subsides, a follow-up treatment may be needed to refine your result.

Aftercare:

You may return to normal activities directly after treatment.

Although, you may experience some swelling as well as redness and tenderness at the injection site for a few hours, these symptoms will normally disappear within seven days.

What is Restylane?
Restylane is a safe and natural cosmetic dermal filler that restores volume and fullness to the skin to correct moderate to severe facial wrinkles and folds, such as nasolabial folds. 

What is Restylane made of?
Restylane is the first cosmetic dermal filler made of non-animal-based hyaluronic acid. Hyaluronic acid is a natural substance that already exists in the human body and is used to provide volume and fullness to the skin. The hyaluronic acid in Restylane is a crystal-clear gel called NASHA or Non-Animal Stabilized Hyaluronic Acid. NASHA is biodegradable and fully biocompatible with human hyaluronic acid.

How is Restylane different than other hyaluronic acid-based products?
Unlike rooster-derived hyaluronic acid-based products, Restylane is free from animal proteins. This quality limits any risk of animal-based disease transmission or allergic reaction. Additionally, Restylane has the highest concentration of hyaluronic acid available in a dermal filler.

Is Restylane safe?
Yes. Restylane has been used by dermatologists and plastic surgeons since 1996 worldwide. There is limited risk of allergic reaction. Restylane is fully biocompatible and allergy testing is not required. After your treatment, you might have some redness or swelling. This will normally last less than seven days. Sunbathing and cold outdoor activities should be avoided until any redness or swelling disappear. If you are pregnant, breastfeeding, or under 18, you shouldn't use Restylane.

Can Restylane be used anywhere on my face?
Restylane has been used in more than 1.5 million treatments in over 60 countries to correct a variety of wrinkles. In the U.S. the FDA has approved Restylane for wrinkles and folds like the ones from the nose to the corners of the mouth. Your doctor will consult with you prior to your injection regarding common treatment areas.

How long does Restylane last?
Restylane is proven to deliver long-lasting results. Studies have shown that Restylane effects generally last for about six months! Restylane stabilization technology helps maintain the cosmetic effect until the injected material is nearly gone.

How often should I have Restylane treatments done?
Treatments are typically scheduled two to three times a year. Having a touch-up treatment before the product has fully dissipated may enhance the lasting effect.

What should patients do prior to treatment?
Restylane requires no pretesting, but you should take a few precautions before being treated. Avoid using aspirin, non-steroidal anti-inflammatory medications, St. John's Wort, or high doses of Vitamin E supplements prior to treatment, because these may increase bruising or bleeding at the injection site. Also, if you have previously suffered from facial cold sores, discuss this with your physician. He/She may consider prescribing a medication to minimize recurrence.

How is Restylane different from BOTOX Cosmetic?
Restylane is a natural, cosmetic filler. In contrast, Botox Cosmetic is a drug that blocks nerve transmission and relaxes the underlying muscles. Many patients have been very pleased after receiving treatment with both products.

How is Restylane different from bovine collagen?
Bovine collagen is derived from animals (cows) and requires an allergy test. Restylane can he administered immediately-no waiting is required.

■ MICRODERMABRASION

Microdermabrasion treatments last about a half hour, and with the relaxing application of soothing products post treatment, you can expect to be in the office for approximately an hour.

Procedure:

Most treatments do not require anesthetics. However, an anesthetic ointment may be applied to numb the treatment area before the procedure. In some cases, a local injection may be suggested.

A beam of light from the medical laser breaks up the ink particles that are then absorbed naturally through your system, lightening and, in many cases, completely removing the unwanted tattoo.

The feeling of the laser on the skin has been compared to that of a snap of a rubber band or a sting that is followed by a sunburn-like sensation.

Directly after treatment you can expect some swelling and/or redness. Your skin may appear discolored or white (hypopigmented), which is normal and will disappear over time. In the case of hypopigmentation, normal skin tone should return in six to 12 months after your last treatment. The treated area may also blister, scab, peel or swell directly after your procedure, depending on the color of ink being removed.

Your treatment will take from 5 to 10 minutes although larger tattoos may require more time.

Aftercare:

Care may vary depending on your skin's response to treatment. It is important that you do not irritate the treatment area as it will be sensitive afterward. If you do scab or blister we recommend you use a topical ointment such as vaseline, bacitracin or polysporin. Otherwise no specific care is needed but you should cover the areas to avoid sun exposure.

Results:

The results from Laser Tattoo Removal treatments can be quite dramatic and some tattoos will disappear completely. Large or heavily pigmented tattoos applied by a professional tattoo artist will probably require multiple treatments, so talk to us about the process. As long as you know what to expect going in, we can probably get your tattoo out.

Precautions:

Due to the fact that there is no regulation attached to tattoo ink, results may vary. As it is impossible to know precisely what material has been used in every application, some tattoos may be partially removed even after several sessions.

■ THE ONE-HOUR FACELIFT

Step by Step Mini Facelift:

1.  Premedication is generally given to relax the patient.

2.  Local anesthesia utilizing the revolutionary Tumescent technique, is administered to insure the patient experiences minimal discomfort throughout the procedure.

3.  Dr. Seiger makes an incision in the natural crease of the skin. The incision originates at the temple inside the hairline, above and forward of the ear and continues around the front of the earlobe.

4.  Dr. Seiger lifts the skin outward and tightens and repositions the underlying muscles and tissues.

5.  At this point, extra skin and fat may be removed.

6.  The incision is closed with the smallest possible sutures.

What is the Thermage procedure?

Varicose veins are a very common condition affecting approximately 80 million Americans. They affect an estimated 40-50% of the female population and 20-25% of the male population. The most common factors leading to venous disease is heredity, age, gender and pregnancy. Other factors may be prolonged standing, obesity, physical trauma, hormonal changes, and sedentary lifestyle. Varicose veins usually appear as bulging or twisting rope like areas on the legs and are known to be a true medical condition, not a cosmetic issue. Why Venous Disease Occurs
Varicose veins occur when veins are not properly returning blood from the lower legs to the heart. The veins have valves that open and close to allow the flow of blood. When a valve malfunctions, blood flows in the wrong direction (backwards) and pools in the leg veins, which results in the bulging of the leg veins. When the blood flows backwards, it is called venous reflux. The faulty valves are usually on the upper leg, even though it is common for the varicose veins to be on the lower leg. Varicose veins will commonly continue to enlarge and worsen with time when there is positive venous reflux.

*An important issue for patients to remember, because venous disease is a chronic condition, it requires on-going maintenance and treatment.

Ways of Treating Venous Disease
We attempt to relieve your symptoms in the easiest way possible. Therefore, we first try preventative techniques and conservative management such as:

• Compression Stockings
• Elevating Lower Extremities

• Avoiding Long Periods of Standing or Sitting

If your varicose veins do not respond to these techniques and if traditional Sclerotherapy is not suitable for you, one or more of the following active treatments may be required:

Endovenous Laser Ablation
This is a state of the art, minimally invasive procedure to treat venous insufficiency. It is safe, fast and effective.  This is done in our office under local anesthesia. A thin catheter is inserted into the vein through a small puncture near the knee using ultrasound guidance. A laser light guide is inserted through the catheter and laser energy is used to seal the faulty vein so that blood can no longer flow through it. The vein becomes fibrous after the treatment and will be dissolved by your body. The blood in your faulty veins will be diverted to your normal healthy leg veins. Following the procedure, you will leave our office with compression hose / bandaging for 4-7 days. There may be a tight feeling in your leg for a few days following the procedure. These are a few of the benefits of endovenous laser:

• Treatment is well tolerated with minimal discomfort
• The procedure takes only approximately one hour to perform
• There is only minor soreness, bruising or swelling
• There is a very high success rate of 93-98%

Ambulatory Phlebectomy
This is a minimally invasive surgical removal of larger surface varicose veins. This is done in our office and often used in conjunction with endovenous laser ablation. It is done under local tumescent anesthesia. The punctures are tiny and the veins are removed through these punctures. A small suture is applied and scarring is minimal. After the vein has been removed, a compression bandage / hose will be applied for 5-7 days. You will be scheduled for a week follow-up for suture removal.

Ultrasound Guided Sclerotherapy
This procedure is performed to treat varicose veins that may be resistant to traditional Sclerotherapy or smaller varicosities that may remain below the surface of the skin after the initial treatment with endovenous laser or ambulatory phlebectomy. This is done while the patient is lying flat and the image is viewed on the ultrasound screen. A fine needle is guided into the center of the varicosity and a precise amount of sclerosing solution is injected into the vein. This seals the smaller vein, no longer allowing abnormal blood flow. You will be wearing compression hose / bandage for 2-4 days and you will be asked to walk after the treatment. Multiple treatments may be required.

Multiple treatments are necessary when treating spider veins. Spider veins are the most difficult type of veins to treat; therefore even re-injection in the same veins is usually necessary.

Every patient heals on an individual basis; some will heal much slower or quicker than others. This also makes the number of treatments variable with each patient. Some patients even with the simplest veins may require up to 3-10 visits to get good results. Before beginning your treatment; you should know treating spider veins is not a quick process, it may take weeks to many months for veins to fade.

It’s very uncommon to achieve any results with only one treatment. After each treatment, veins may even look a little worse while they are in the healing phase. We can continue to treat your veins as they are healing; we can easily see which veins are healing and closing off.

Treating your spider veins may produce some side effects for short periods of time. Any or all of these side effects may occur:

• Blue or dark discoloration (pigmentation); this may take up to 2-12 months to fade
• Bruising; which will take 1-3 weeks to fade
• Small lumps that feel hard under the skin (hematomas); these usually resolve on their own over a 1-3 month period,
• Small red, raised bumps at each injection site; these will usually resolve by the following day and they may itch a small amount,
• Ulceration; this is a small open sore - the risk of an ulcer is rare, but can occur at any injection site. Ulcers are benign and heal on their own after a few months. They may heal with a scar.

Normal spider vein injections can rarely produce an occurrence of new spider veins (matting) in the area near the injection site. These areas can also be treated along with your regular treatment.

The smaller blue veins (reticular veins) can also be treated with sclerotherapy. Treatment of these veins will help with the reduction of some spider veins. The rare side effects of treating these veins can include discoloration and sometimes production of new spider veins (matting).

A treatment will consist of a variable number of injections each time you have a session. Your treatments are not based on a certain time period, but on the amount and percentage of solution used. A trained nurse or medical assistant may be treating your spider veins.

Common questions regarding Sclerotherapy

What happens in the first session?
During the initial session, the procedures and all information is reviewed. A test dose of the solution is given at this time to determine possible allergic reaction. If testing such as a Doppler is required, an appointment will be scheduled at this time.

When do the veins disappear and do they return?
After injection, the vessel usually disappears in 4 to 8 weeks. They do not recur after proper treatment, but this treatment does not prevent new veins in the future. Some veins require multiple injections.

How often are the treatments?
Usually treatments are performed bi-weekly until completed, but can be varied.

What other forms of treatment exist?
Other treatments consist of laser, electrodessication & surgery.

What can you expect after treatment?
You will have a large amount of bruising. You may have small hard lumps at the injection sites. These lumps will fade in a few months. You may also get some discoloration on the skin, this will fade.

Questions and Answers

Can all kinds of veins be treated?
Yes. Large veins, spider veins and facial capillaries can all be safely treated.

How many injections will I be given during each visit?
The number of injections will vary. It’s not the number of injections that determines the effectiveness of the treatment, but the type of sclerosing agent used, its dosage and the site of the injections.

Does the treatment involve any risk?
All medical treatments entail a certain degree of risk. This will be discussed during your first visit. Fortunately, when sclerotherapy is administered appropriately serious complications rarely arise.

Is the treatment painful?
Pain may sometimes be induced by bruising, but is usually minimal. The small needle causes minor discomfort.

Will I have to rest in bed after each treatment?
No. You will be able to return to your normal activities right after you leave the office. In fact, normal activity and exercise is strongly recommended.

Will I have to return to the office after my treatment program is completed?
We suggest you come in for a follow-up visit once or twice a year so that we can check for any new varicose or spider veins.

Will I leave the office with my legs bandaged?
Yes. In some cases the patient will also be required to wear special support stockings. You may be required to wear the bandages anywhere from 12-72 hours.

Should I wear support hose after the treatment?
We suggest them for anyone suffering from venous congestion. They should definitely be worn whenever you have to stand for long periods of time. It will be recommended for most patients with venous insufficiency.
